Get vaccinated against COVID-19 for a shot to win $250!

Two debit cards of $250 each will be raffled off on the evenings of December 13 and 20 during clinic hours (5 p.m. – 8 p.m.). Moderna & Pfizer vaccines will be available for both adults and children.

Each person receiving a vaccination will get one entry. You can get a second entry if:

• It’s your first dose of the COVID-19 vaccine, or
• You bring in an individual that gets a first-dose vaccination (each person you bring earns you 1 additional entry!).

How to Enter

Anyone who wants to be entered in the raffle for a $250 debit card must register using the forms below. One form is for children ages 5 and 15, and the other is for ages 16 and older. If you bring a friend, they can also register and get a chance to win too!

The registration forms will remain open until the end of each clinic night. Participants will be entered into the raffle held on the night that they receive their vaccine.
